中国的传统文化是以儒家思想为核心，历来主张尊卑有别，长幼有序，并受到等级观念的制约。而美国文化主要承袭了基督教中的新教思想，等级观念淡薄。他们更注重个人权益，追求人人平等，没有辈份间过多的礼节。在《刮痧》许大同自己发达以后，将老父亲接到美国照顾，这是典型的中国人的思维，虽然父亲对新的生活不能适应，但内心还是非常幸福的，并且认为这样自己可以好好地孝敬父亲。

时空上的差异造成了中美—东西方文化、观念上的不同，进而表现为行为上的冲突。差异性从时间上来说，主要表现为国家的传统与现代之间的矛盾；从空间上来说，由于各国在地域、历史、政治、经济等方面的原因造成了中西方的差异，表现为民族传统精神背景、文化背景、哲学背景等方面。中、西两种文明之间存在文化差异是可以理解的，也是不可避免，要消除这种文化隔阂，就必须要相互增进了解，只有这样才有可能消除误会，正如周总理所说我们要求同存异，相信“无论是怎样的文化沟壑，只要相互交流就能达到谅解。”
